During a right hemithyroidectomy, the patient was found to have a suspicious lump alio loco in the area of the carotid bifurcation. This was extirpated during the same session. The external pathohistologic examination revealed a metastasis of a squamous cell carcinoma. In further external diagnostics, the primary tumor in the area of the lingual epiglottis on the right could then also be verified histologically. There is now an indication for transoral resection of the tumor and neck revision on the right side. The patient had ample opportunity to ask questions about the procedure before the operation.
