FINDINGS:
PA and lateral views of the chest provided.   Left chest wall AICD is again
 noted with lead extending into the right ventricle region.  Hilar congestion
 is noted with small right pleural effusion and subtle retrocardiac opacity
 which could represent subtle pneumonia in the correct clinical setting.  No
 pneumothorax.  Heart size is mildly enlarged.  Mediastinal contour is normal. 
 Bony structures are intact.  No free air below the right hemidiaphragm.

IMPRESSION:
Mild cardiomegaly with hilar congestion and no frank pulmonary edema.  Small
 right pleural effusion with subtle retrocardiac opacity concerning for a
 possible right lower lobe pneumonia.
