FINDINGS:
There are low lung volumes, which results in bronchovascular crowding.  There
 is no focal consolidation, pleural effusion, or pneumothorax. There is
 bibasilar atelectasis.  Heart remains enlarged.  Large hiatal hernia again
 seen.  A nasogastric tube tip ends in the esophagus, proximal to the hiatal
 hernia.

IMPRESSION:
Nasogastric tube tip ends in the esophagus, proximal to the large hiatal
 hernia.
 
 RECOMMENDATION(S):  Advancement of nasogastric tube.
