FINDINGS:
Evaluation is slightly limited by patient rotation. Patient is status post
 median sternotomy and CABG. A right-sided pacemaker device is noted with
 single lead terminating in the region of the right ventricle. Additional
 abandoned lead is noted within the right anterior chest. Moderate enlargement
 of cardiac silhouette is re- demonstrated.  Mediastinal and hilar contours are
 grossly unchanged. There is mild pulmonary vascular congestion with small
 bilateral pleural effusions. Bibasilar atelectasis is also demonstrated. No
 pneumothorax is visualized.  Extensive degenerative changes are noted
 involving both shoulders. There are multilevel degenerative changes also
 identified within the imaged thoracic spine.

IMPRESSION:
Mild pulmonary vascular congestion and small bilateral pleural effusions with
 bibasilar atelectasis.
