FINDINGS:
There is stable marked enlargement of the heart with mild pulmonary
 vascular congestion without overt edema.  Retrocardiac opacity with subtle air
 bronchograms could reflect left lower lobe pneumonia.  Small left pleural
 effusion cannot be excluded.  There is no pneumothorax or right pleural
 effusion.

IMPRESSION:
Possible left lower lobe pneumonia. If clinical status permits, PA
 and lateral radiographs would allow for better evaluation.
